COVID 19 POLICIES
For the protection of our patients and staff we ask that all patients adhere to the following policies during your visit to our practice.
Patients are required to wear face masks during the entirety of the visit. If you do not have a mask, one will be provided.
Please wait for a staff member to greet you before your entry to our office. If you are waiting for another patient to enter please observe social distancing rules and stay at least 6 feet away.
Only scheduled patients will be allowed into the office, no other persons will be admitted unless accompanying a patient under 18 or assisting a disabled patient.
All patients are required to consent to a contactless temperatures check. Patients with elevated temperatures will not be admitted and will be asked to reschedule until they have been symptom free for at least 14 days.
All patients are required to wash their hands or hand sanitize upon entry.
Patients are required to notify our staff if there has been contact with a person who has tested positive for Covid 19 in the last 14 days, or have experienced any of the following symptoms: fever, chills, shortness of breath, muscle pain or weakness, symptoms of the flu, cough, runny nose, rash, sore throat, loss of taste or smell, or any Gastrointestinal problems such as nausea, diarrhea or vomiting within the last 14 days. These patients will not be admitted and will be asked to reschedule until they have been symptom free for at least 14 days.
Our bathroom is currently unavailable for patient use. There are public restrooms available in the Scottsdale Quarter located on the north side of the Quad or on 72nd Place, next to The Gap.
As social distancing is impossible during direct ophthalmoscopy, we are requiring all patients to have a retinal photo in lieu of dilation. This may or may not be covered by your insurance company. Please ask our staff for more information.
